Request to Use a Commonly Used First Name and/or Remove a Title
Step1
Student
- Review and download the application form from the Faculty of Sciences website
- Notify the Cross-Functional Missions Division at fds.mitra@umontpellier.fr of your request
- Fill out the form, sign it, and contact the VP RS: vprs@umontpellier.fr
Step2
Vice President of Corporate Social Responsibility (VP CSR)
- Meets with the student and provides support
- Sign the form and give it to the student
Step3
Student
- Submit the completed and approved form to the Cross-Functional Initiatives Division fds.mitra@umontpellier.fr
Stage4
Cross-Functional Missions Division (MITRA)
- Issue the new student ID card and give it to the student
- Notify the student of their new UM email address
These procedures allow for the use of a preferred first name only on documents internal to the institution (e.g., student ID card, attendance sheet, email address, etc.).
To amend official documents (diplomas, contracts, agreements, etc.), you must obtain a notification of authorization to change your first name—a process handled by the registrar: service-public.fr
Request to Use a Common Name and/or Omit a Title
Please fill out and sign the application form below before submitting it to fds.mitra@umontpellier.fr, along with a document proving your right to use the name (copy or extract of a civil status record, photocopy of the family record book, national ID card, etc.).
Application to Change a Family Name
Please submit your current National ID card or a new birth certificate to fds.mitra@umontpellier.fr. The Registrar’s Office will update your file and your institutional address. You will also be issued a new student ID card.
