Request to use a first name and/or remove the title
Step1
Student
- Read and download the application form from the Faculty of Science website.
- Notify the Cross-Functional Missions Department at fds.mitra@umontpellier.fr of your request.
- Complete the form, sign it, and contact the VP RS: vprs@umontpellier.fr
Step2
Vice President of Corporate Social Responsibility (VP CSR)
- Meet the student and accompany them
- Fill out the form and give it to the student.
Step3
Student
- Send the completed and signed form to the Cross-Functional Missions Department fds.mitra@umontpellier.fr
4th stage
Cross-Functional Missions Division (MITRA)
- Issues the new student card and gives it to the student.
- Inform the student of their new UM email address.
These steps allow for the use of the first name only on internal documents within the institution (e.g., student ID card, attendance list, email address, etc.).
To amend official documents (diplomas, contracts, agreements, etc.), notification of authorization to change your first name is required. This procedure must be carried out with the registrar: service-public.fr
Request to use a common name and/or remove a title
Fill out and sign the application form below before sending it to fds.mitra@umontpellier.fr, along with proof of your right to use the name (copy or extract of civil status certificate, photocopy of family record book, national identity card, etc.).
Request for a change of surname
Send your updated National Identity Card or a new Birth Certificate to fds.mitra@umontpellier.fr. Your school will update your file and your institutional address. You will also be issued with a new student card.
